Someone I know could not be approved for insurance coverage because: Minor height/weight variation. Does anyone know what that would mean?

I don't think it's your height and weight that you didn't get coverage. You need to ask the insurance company rejected you for an explanation why they turned you down.

By using the BMI calculation (body mass index), the person has a score of 26.09.

Under 18.5 is considered Underweight
25 to 29.9 is overweight and
above 30 is Obese.

Since about 80% of Americans are in the 25 and over group, you shouldn't have too much of an issue getting insurance. Just ask for a physical if they give you any grief.

Is the person male or female? That will make a difference as will their age. If male there shouldn't be any problem but some companies will not insure a female of that height.

Have they gained or lost a lot of weight in the past year? If so, that may cause a company to decline as well.

You're best bet would be to contact the agent for more information. They agent will know why they were declined. If the agent is independent they can find another company that will accept this person.

If someone is 6'5" and weighs only 220, I would think they would be very thin. If they are in good health, no diabetes, high blood pressure, etc..there should be no problem in obtaining health insurance.
