It sounds as though your cat is stressed about some change in your life. Have you moved recently? Gotten another pet? Is another person living in your home now? If all of these are answered 'no' then I'd recommend taking your cat to a vet for a checkup. She may just need a light sedative for a week or so until she's back to normal. Good luck with your kitty!
*** Also if she has peed on anything washable, put a cup or so of plain vinegar into your wash and it will remove the smell. If the cat can't smell the pee she is less likely to mess on your laundry or stuff again. In addition make sure the litter box is cleaned several times a day. Cats will often go outside of the litter box if they think it is dirty. Not only scoop the litter, but completely change out the litter and wash the litter box before putting in the fresh litter.

Take her to the vet if you're worried!

Number one reason for scratching and hair loss is fleas. If you're not regularly treating her, or you're using something from a pet store/supermarket, she's likely to have fleas and probably an allergy to them. The peeing on things could be behavioural, has anything changed in the house lately?

If you need help with an animal, the vets is the place to go not really Yahoo Answers because people don't really know what they're talking about.

A vet is the best person to help your cat with both matters. The skin problem could be anything from parasites, allergies, dietary problems or even severe stress.

Is she spayed? If she's in heat she may be scent marking her territory. Urinating around the home can also be the sign of a bladder or urinary infection.

In the meantime, the web articles below offer information and advice that may be helpful regarding both problems.

Sounds like she has a skin condition. Mange? Fleas? You would have to take her to a vet and get it diagnosed.
