Assalamu'alaikum.
Aged person who cannot even fast even later. The answer is "Fadiah". In Malaysia I Cupak uncook rice (beras) daily. Times the number of days and give away to the poor.
The other question is other not keeping fast in Ramadhan. In My Country Malaysia, there are enforcement officer who will punish in Syariah Court for those who are not Fasting that been caught. The penalty will be fine and jail. For the Fasting that they did not perform, the Islam law said it must be replace. They have to replace all the days they are not Fasting.
The third question, who cannot even fast even later. If they are young, they can fast. They must practise. example. 1st day. Perform Half (1/2) day of Pasting. 2nd day. Three Quarter (3/4) of the Day. 3rd day Full day. To my knowledge, until today, there is nobody die in performing fasting. My daughet age 7 Fasting from 1st Ramadhan until today. It's temptation. It's willpower. Have a motto. "I can Fast, If I think I can". If others can Fast, why can't I fast.
Bare in mind, the prize of fasting is personally graded by ALLAH SWT.

I cannot fast now, or ever. I am an insulin dependent diabetic who can have severe diabetic complications if I fast for Ramadan. Instead I pay a 'ransom' to a poor person by buying their breakfast every day during Ramadan.
For those who think there is NO excusable reason to not fast, you must be fortunate enough to enjoy good health. Consider yourself blessed in that regard.

You could make it up by giving charity and feeding the hungry. This is allowed for any reason you missed a fast.

If you dont fast and your between the ages of 12-80 and you aint sick or pregnant there aint a reason for you to not fasting.

My husband told me that old people, young people under the age of puberty, the sick, pregnant women, breastfeeding women, and women on their period don't have to fast. But women on their period have to make up for the days they miss due to their period. My mother in law is really old and she still fasts.

It was always my understanding that there are exeptions to the Ramadan fast, for children, the elderly, the ill, and so on.
