I have always wondered why. Does it hurt or is it from pleasure?

It is supposed to mean pleasure, but could signify pain too.

This is why communication with your partner is an essential part of any relationship. If it feels good, do it. Of course, there are those few out there who are into the pain.

I guess it kind of depends on the situation and the players.

Sometimes it is because your partner expects it. Kind of like you laugh loud when you are in a group watching a comedy but when you are alone you just kind of smile and chuckle. It is a socially expected response in a lot of ways. Sometimes your eyes just roll back in your head and you just suck in a breath and sigh. It's sometimes just too good. When you get kids you kind of hide under the covers and do that, so you wont attact their attention or wake them up. Some women feel pressured to moan and cry out because their partners get more excited, and sometimes they really aren't feeling much but I think most women would admit to themselves that they are sometimes "faking it" but don't tell them I said so.

The first time might hurt, and most of the time it isn't so great, it can be scarey or embarrassing. You get over that pretty quick.

Both... it really depends, there are some times when it can hurt, but that is not the normal case and can be generally avoided. Some times pain can be pleasurable, but if it's not for you, then let your partner know so that you can avoid it.

It's interesting but the yelps/sounds that people make change in difference cultures and societies...

It's both - a pleasure thing, similar to getting a massage (only better). Or it could be pain (which, generally you'll be able to tell the difference judging by the look on her face). But then again, some people like pain.

Like the guy above me said, communication is key.
