It has a dispenser for oil that can be burnt to make smoke. It has a hot surface, like an electric hot plate, that the oil can drip onto for burning. It also has a fan that can blow the smoke produced out a duct that could be pointed in different directions.

Some are fancier with variable speed fans, more control of oil rate, larger tanks, a flame based hot plate, and better paint jobs on the machine. But the above is the basic idea.

It is a heating element that heats glycerine, the fluid that you place into the machine. Glycerine, when heated, produces a fog that is not toxic.
