There are some phrases in Italian, but this is mostly in Sardinian, which is not my strong point. Anyway, I'll give it a go to the best of my ability:

[the first 4 lines are a dead loss]

This isn't all in Italian, I don't know which language it is...the sentences who are in Italian I will translate you:

E’ la vita mia = It's my life
E t’amo, e t’amo (e ti amo) = and I love you
Ogni cosa tua = each thing of you
Sento le parole tue = I hear your words
Ogni nota tua = each note of you
Anche se non è più casa mia = also if it isn't anymore my home
Sento le tue melodie = I hear your melodies

Sorry, but the other things aren't in Italian. Could it be that it is a song and you wrote it down, such you heard it?

Assuming you're speaking to a minimum of one person ("Siete" is plural) actually it would be "Sei nei miei pensieri. Incondizionatamente". In thought is real, yet purely "does not artwork" in Italian, especially "unconditionally" would sound fairly unusual to assert. you may say: "Sei sempre nei miei pensieri" ("you're continuously in my recommendations") "Sei sempre a comunque nei miei pensieri" ("you're continuously, any way, in my recommendations", the meaning is closer to "this is no longer suitable if I had to think of to something else, you're continuously in my recommendations besides")
