Poor handling of food at home. You hear in the news about Salmonella in Peter Pan peanut butter, and bagged spinach w/ E. coli. As bad as those items are, there are undoubtedly tens of thousands of cases of mothers, fathers, brothers, and sisters that mis-handle or undercook food. You never hear about it in the news, but it happens all the time.

I cannot say this is the #1 answer, but I know botulism in canned foods is a big one. From what I understand you cannot smell or taste it. If you suspect it...dented or bulged cans...don't chance it...throw it out!

Poultry. I think. then seafood.
Non Kosher meat/beef. Where e coli forms. Not being cooked long enough nor hot enough degrees.
