1. Say the Shahadah
2. Learn and follow the five basic pillars of Islam
3. Try to go in depth and learn as much as you can about Islam and practice it as devotedly as you can

BTW, you might want to pop by Saudi Arabia for a few years. Living here really helps you appreciate Islam and see it in a different light. Of course, there are many things that are done incorrectly here but still, it is the birthplace of Islam after all.

You say... I bare witness there is no deity to be worshipped but Allah SWT(The Only God), and his messenger and final prophet is Muhammed Peace be unto him.
its as simple as that, take a shower and wadu (abultions) first inshAllah tahala
You also say, Mary a virgin peace to her was the mother of Jesus and Jesus was her son. Salam :)

To become muslim, you simply must beleive In the angels, heaven, hell, his holy books, all his prophets and most importantly: 'there is no god apart from God, and Muhammad is the last Messenger of God.' By this declaration the believer announces his or her faith in all God's messengers, and the scriptures they brought

Some Muslims acknowledge other events from Islamic history, which are considered holidays by some but not all Muslims:
Islamic New Year: 1 Muharram
Ashura: 10 Muharram
Mawlid an-Nabi: 12 Rabia' Awal
Isra' & Mi'raj: 17 Rajab

You would have to study the Qu'ran, (preferably learn Arabic) with a Muslim. After you study, it you take the Shahadah, which is kind of like a pledge stating that you don't worship idols, or humans, just God and God alone. You do this with Muslims, so they are kind of a witness to your conversion.
You will observe the holy days and Months that Muslims observe such as Ramadan. You will refrain from eating pork, and you will pray 5 time a day, also you will have to give to charity, also make your pilgrimage to Mecca, once in your lifetime.

To become a Muslim one must recite the Shahada, which is the declaration of faith:
"I bear witness that there is no deity (none truly to be worshipped) , but Allah, and I bear witness that Mohammad is the messenger of Allah"
