It is possible that whatever mechanisms are responsible for interrupting dreaming in case of a biological need (such as hunger, thirst, or the need to eliminate) are underdeveloped for some reason, or the mechanisms responsible for keeping a sleeping person asleep in spite of interruptions are overactive.
As for a solution, I would recommend trying one of the devices designed to help people with problems like this, which detect the first few drops of urine that usually come before all-out urinating, and then vibrate or buzz to awaken the sleeper. Through classical (sometimes called Pavlovian) conditioning, this tends to lead to awakening as soon as the urge to urinate becomes strong enough, before the sleeper urinates in bed.

The sad thing is, I knew you were going to say this! lol
Yes... only once, when I was 19.
I always thought it would be really bad to need to go and dream of going...and once I actually did just that. Woke up and stopped myself, but it was already too late!
Glad to know I'm not the only one that has. lol

According to Dreamcrowd:
To see urine in your dream, signifies feelings you have rejected. To dream that you are urinating, symbolizes a cleansing and release of negative or repressed emotions. Urination is symbolic of having or lacking basic control in your life. To dream that you are urinating in public, symbolizes a lack of privacy in your affairs or your need to make a public apology or confession. See also: Urination
To see a toilet in your dream, symbolizes a release of emotions or getting rid of something in your life that is useless. If you are cleaning the toilet, then it means that you are starting to shed your shell or lose your inhibitions. To see a clogged toilet in your dream, signifies that you are holding in and keeping your feelings to yourself. Your emotions have been pent up too long. To see an overflowing toilet in your dream, denotes your desires to fully express your emotions.

Yes, it has happened to me quite a few times although not in the past 10 years. I don't know why it happens - but it stopped.

This has happened to me every once in a while, but only when I was little. I suggest you find that medication that helps bladder problems if you feel really insecure about it.

Try going to the bathroom before you go to sleep and not drinking so much.
