Do you have another job (jobs are not easy to come by anymore) and do you want a good reference in the future?
Put it in writing, leave your personal (bad) feelings out of it and be professional.
Dear Jan:
Thank you so much for the opportunity to work with you and learn your business over the past 5 years. Regretfully, I have decided to pursue other opportunities and my last day with XYZ company will be on September X, 2007. (make sure it is at least 2 weeks away).
I have enjoyed working for XYZ company and know that the skills I gained here will assist me throughout my career (even if it is bs).
I wish you best of luck in all your future endeavors,
Me.

I would just talk to my boss directly but if you write a letter just say something like I enjoyed my time with the company but I was offered a position in another company that offers more room for growth better benefits and higher pay. So I am putting in my 2 weeks notice to resign. :) Thats what I would say

I agree with Gem (and her sample letter is great). Write a letter, keep it very professional, you don't want to burn your bridges (maybe one day you'd like to return to the companY), and you'd like them to provide a good reference if necessary. Definitely give two weeks notice if possible. I used to work at a bank and can't tell you how many people just left without notice and put us down as a reference (once the new company calls for the reference we simply state that we would not rehire them). If you and your boss have a good rapport, you may want to speak to them first before you write the letter (its what I did, and my boss was very happy for me and appreciated me telling him personally first, instead of handing him a letter first...I did both).

short and to the point.
Dear Mr jones,
I wish to resign my position with this company effective Friday Sept 28,
Thank you for the opportunities which have been extended to me.
Very truly yours,

Don;t go any further. Probably, your boss will want to talk to you about it, and at this point you can go into details.
Basically all this letter does is make it official you are leaving on your own.

Explain what a meaning position you held at this employment and how it has open doors for you. Then say you need to explore other avenues to grow and thank the boss for his superior leadership mentality (if you like him).

It doesn't matter the contents or reasons behind the move but keep it short and sweet... My last day will be this and thank you for the opportunity to work with you and the company...
