Basically, it's very much like any other P-N junction, only the junction area is usually a bit smaller to minimize junction capacitance. Some of them are also optimized for low forward voltage drop.

It depends on what you mean by a high-frequency diode. If you are talking about a high-speed switching diode, then obviously that is its purpose - switching at high speeds.

Too, there are diodes for microwave applications, which are much faster. Some of these are used in microwave frequency multipliers and such.
