In the UK if the marriage has broken down and you want to revert to your maiden name, you just phone the DWP and tell them your marriage has broken down and you wish to be known by your former name. Its has simple as that, as long as you wont be using your maiden name to commit fraud that's okay.

If you are getting a divorce, you can stipulate that you want to change back to your maiden name. If you are already divorced, what you need is called an adult name change--and you cannot get that from Social Security as it is a legal issue.
Go to Google and type in "adult name change" and your state's name. You will find that there are folks who will charge you for the application. If you look further, you will find it for free--offered by your state or its bar association. You fill it out--and you must list every name you have been known by--including nick names, and family information, etc. You will need your birth certificate. Then take the app. back to the clerk's office or your local government center and pay them a fee for filing and search of about $200.00.
It can take as long as 90 days to be processed. You will be called and given a time and place for your hearing in front of the judge. You will be asked to swear or affirm to the info on the app. If the judge approves it, your name change is effective immediately.
Be sure to get at least 5 notarized copies of the decree--THEN you go get your driver's license changed--take a copy of the decree, and the same for Social Security, voter's registration, insurance, etc. If you don't do this and use a different last than your married name, you will be using an alias.

If you are in the UK you can officially change your surname through Deed Poll all you have to do is apply on the net at www.deedpoll.co.uk it is very easy and is explained very well hope this helps

You do it the same way that you changed to your married name. Get a copy of your divorce decree and apply for a new social security card, drivers license, checking accounts and credit cards. The bank and credit card companies might have paperwork that they need like your new social security card/ ID card. Don't forget to change stuff like your passport or investments, insurance policies, 401k's, you have to do it all or it is a hassle later!
