It's not a big deal to replace a lost or stolen passport, just go to your County Clerk's office and tell them your's was thrown away and you need to replace it.
I don't know if it's the same everywhere but it it $68, plus a $30 processing fee.

The following answer applies to a US Passport only.

Note, you are supposed to report a lost or stolen as soon as your passport is lost or stolen, and you should do this right away. The US Department of State provides a phone number and form with which to do this.

There is a specific form for a lost or stolen passport (my son has lost his passport before), and you have to fill it out explaining what happened. You do this along with the passport application. Both forms have to be submitted together.

The fees and documents required are the same as for a new passport - and you cannot apply by mail, you must apply in person.

I believe the fees are the same as a new passport also. As someone else said, about $90. Plus more if you want to expedite it.

Here's the state department's page on this in case you want to see more details.

You will have to reapply for your passport. The first step is to report it as lost or stolen. After that, you will fill out the form DS-64. You will need to go in person to a Passport Agency or Acceptance Facility. The department of state website will walk you through the procedure; you can find your closest center as well as download the required documents. Here is the link: http://travel.state.gov/passport/lost/lost_849.html
Best of luck with getting your passport. Processing time is now taking anywhere between 10-15 weeks.

I had an expired ID card stolen, (here they are classed with passports and can be used instead of a passport for European travel.)
I had to report it as stolen, take the paperwork of the police with me to the passport desk in our town hall (that is the normal place to get a new passport here,) and pay an extra fee for a new one.
For a passport the thing would have been the same. I could identify myself with my passport when getting the new ID card.
If both had been stolen at the same time, or if I had not had a passport I would have had difficulties proving that I was me, to get a passport or ID at all.

There are two steps required to get a new passport. First, you must report your valid passport lost or stolen immediately. This is required of anyone who loses or has a passport stolen. The second step is to replace your passport. Both steps and forms required are described below.

To Report a Lost or Stolen Passport:
Complete Statement Regarding Lost or Stolen Passport, Form DS-64

Fill in as much of the passport information as you can.
Answer all the other questions in detail.
Sign and submit above Form DS-64, Statement Regarding a Lost or Stolen Passport, to:

U.S. Department of State
Passport Services
Consular Lost/Stolen Passport Section
1111 19th Street, NW, Suite 500
Washington, DC 20036
Do not mail your new passport application to this address. In order to obtain a new passport you must appear in person at a Passport Agency or Acceptance Facility as outlined below.

Or call us toll free at: 1-877-487-2778 (TTY 1-888-874-7793)
Operators are available 7 a.m. to midnight, ET, Monday-Friday, excluding Federal holidays.

IMPORTANT NOTICES

If your still valid passport was lost or stolen, please submit the DS-64 form with the DS-11 application when you apply for a replacement at a Passport Agency or Acceptance Facility. NOTE: You must appear in person.
The information you provide on the DS-64, Statement Regarding a Lost or Stolen Passport will be entered in our Consular Lost/Stolen Passport System.
Passports reported lost or stolen are invalidated and can no longer be used for travel.
If you recover the passport after you have reported it lost or stolen, please submit it to the address listed above. When you submit it, if requested - we will cancel it and return it to you. If not requested, it will be destroyed.
Once a passport is reported lost or stolen, it cannot be re-validated.
To Replace a Lost or Stolen Valid Passport:
Complete Application for Passport, Form DS-11

You must appear in person at a Passport Agency or Acceptance Facility.

Complete Question #18 as follows:

Write your name as it appeared in your passport.
Write the approximate date of issue.
Include the passport number if known.
Under "DISPOSITION", mark the appropriate box.
If your passport was expired, write "EXPIRED" next to the "OTHER" box.
Submit your form:

To a passport acceptance facility.
You will also need photos, documentation and fees. See How to Apply in Person.
NOTE: If you travel extensively, you may request a larger, 48-page passport at no additional cost. To do so, please attach a signed request for a 48-page passport to your application.

Call the passport office first and see if they will replace it. Most likely however you will have to apply for a new one.

Cost is $90.00 and takes 10-12 weeks.

Throwing away a passport isn't against the regulation. Stealing a passport (or the rest) is against the regulation, no count if that's thrown away or no longer. in spite of if, looking something in you domicile and removing this is no longer stealing.
