If you mean to keep up with the donation you make to your church, simply write your name on the envelope provided at tithe. They very rarely don't have them.

They need to include wording similar to this:

Thank you for your cash contributions totaling $70.00 that St. Swithun's Anglican Church received for the year ending 31 December 2006. No goods or services were provided in exchange for your contributions.
