a. The medication order should include the patient's name and address.
b. The medication order should include the medication name and dosage.

The answer is (a).

The medication order written by a doctor does not need to include the patient's address. Just the pt's 1.) name, 2.) route, 3.) time, 4.) dose, and 5.) medication.

Alls I know is that A. thru D. have to be right so E. has to be wrong even though I don't know what E. is talking about so it's just a process of elimination!
The Pharmacist needs to put a name and address on the medication so he/she and the customer know they are getting the right stuff....also the type of medication needs to be printed on the bottle so the client knows they are taking the right medication....the patient also needs to know how the medication needs to be taken, by mouth or in another route...and last the patient needs to know when and how often the medication should be taken! So there you have it!

A through D are most definitely required. E is dumb...very very dumb, for one medication as a standing order would require a complete and utter lack of caring on the doctors part. A standing order for medication is actually pretty funny it makes me think about the movies when you see the doctor hand the patient a random drug that he/she has no clue what it is and just takes it for the fun of it, and standard protocol, man JHCAO would love to hear that

E . The medication is always SPECIFIC Standard protocol for a medicine does not exist.
