I think I may have a problem. A while ago I developed some muscle tension in my neck and head, and I went to the Physical Therapist for it a few times and he resolved it. But I forgot to mention that in my medical screening when I processed for the Air Force. so now that I'm in, I'm worried about it flaring up in Basic and that they'll get my medical records and see that on there and discharge me for 'fraudulent enlistment.' I guess my question is what should I do? and also can they even look that up? Aren't my records private unless I give them consent to obtain them?

Well to begin with it was an honest mistake and no they can't get your medical records without permission.
If it's not a problem now, don't make it one.
If it fares up again you can always give the record to the doctor or include them in your medical record yourself.
Or just send for them now and explain the situation and have the medics put it into your medical record.

Could that fradulent enlistment scenerio happen ? Most likely not unless you were trying to get out on a medical discharge and get disability pay from the VA for that injury.

Muscle contractures of the neck, spastic or non-spastic, are only disqualifying if they interfere with the wearing of military equipment or are so disfiguring as to be repulsive in normal social contact. Still, I sense you are holding back something on describing your condition. You obviously did not file a complete SF-89 (Report of Medical History) at the time you sought enlistment. However, you also signed that document and in doing so gave consent for the MEPS or the Air Force to obtain your civilian medical records if they need to do so.

I doubt they will be too bothered about some childhood things. If you can't remember, then put no. I suspect they will be far more interested in major things such as operations, on going medical problems etc.

you are talking about muscle tension..not diabetes..there is a huge difference...cause you sprained your ankle when you were six should you have put you have a weak ankle? and wasnt the question do You have any medical records..meaning you in your hands? because i have a friend who told her recruiter a problem she had and that was exactly what he asked her...

So go back to your master sergant or lt col and tell them to u need to put it in, Just say you made a mistake
