This is for Carnival, other mass market lines will be similar.

Guests are required to be 21 years old (on embarkation day) to travel. Guests under the age of 21 must be accompanied in the same stateroom by a parent or guardian 25 or older. The exceptions Carnival will make to this policy are:

Married minor couple - The booking must be documented that the couple is married. The couple must have proof of marriage at embarkation or they will be denied boarding without the benefit of a refund.

Domestic Partners/Same-Sex Union minor couple - The booking must be documented that the couple are legal Domestic Partners/Same-Sex Union. The couple must have legal proof of partnership/union at embarkation or they will be denied boarding without the benefit of a refund.

Guest ages will be verified at embarkation. Guests not conforming to this policy will be denied boarding and assessed a 100% cancellation penalty. NO exceptions will be made at embarkation.

The Group policy is slightly different because of stricter group terms and conditions including mandatory chaperones, damage deposits and other considerations.
Now what is common in the real word for families is to book a parent / child per room and then switch rooms once you get on the ship.

Carnival Cruise line requires that at least one person in each cabin to be at least 25 years old. The policy for other cruise lines is different; they require at least one passenger to be 21 years old.

BUT, you need to know that cruise lines do not sell their cabins as SINGLE OCCUPANCY. That means that all cabin rates are sold based on two people occupying them. Now you can go alone if you want, assuming you meet the age requirement, but the fare will be based on two people. So if the cabin that you select is $800 per person, then you will pay $1600 for it to be in it alone. If you still want to go alone, then pick the cheapest cabin on ship (an inside lower deck cabin), which may be only $500, and thus you will pay less for your single.

Here's Carnival's age policy:

The requirement on Royal Caribbean, Princess and other cruise lines are different. They require that at least one person be age 21 or older.

Here's the Royal Caribbean policy straight from their web site:

"No guest under the age of twenty-one (21) will be reserved in a stateroom unless accompanied by an adult twenty-one (21) years of age, or older. This age limit will be waived for minor children sailing with their parents or guardians in adjacent staterooms or under-aged married couples (proof of marriage is required). Royal Caribbean International reserves the right to ask for proof of age. Royal Caribbean Internationals age policy for gambling and consumption of alcoholic beverages is as follows: Please note: An individual's age on the date of sailing determines his or her status for the entire cruise vacation."

Here's what Princess requires:

"Passengers under the age of 21 years must be accompanied by a Passenger 21 years or older who shall assume responsibility for their care during the cruise. Each Passenger hereby agrees and warrants that he/she will supervise any Passenger in his/her care at all times to ensure all policies, along with all other rules of the Carrier and ship, are strictly adhered to by all Passengers under their supervision. No alcohol may be purchased or consumed by any Passengers under the age of 21. No Passenger under the age of 21 shall be permitted to engage in any gaming activities on board the ship. No Passenger under the age of 18 shall be permitted to purchase cigarettes or tobacco products. Passengers hereby agree not to bring alcoholic beverages of any kind (except wine and champagne) on board for consumption."
