Most jury summons are for district or superior courts, depending on what term is used in your state for the criminal court system. Get out the phone book. Look under the listings for your county government. Then call the clerk of the court that is listed for criminal matters. If you can't find it, call the news desk at your local TV stations and ask them for the number. They have reporters covering those courts and they should be able to tell you.
If you think it was for a Federal court, get out that same phone book and look under "U.S. Government".
Do it in the next day or so. You don't want to ignore your summons (that's what that document was that you lost). If you do the judge will issue a bench warrant for your arrest and anytime you are stopped by any policeman, it will show up in the system.

You can call your local court house and they can take it from there. Just explain your situation to them
