You poor thing!
Make sure you have good posture and don't do too much bending over. Instead of bending over, squat down to reach things that are below your hips. Wear comfy & supportive shoes- no high heels! Ice or warm packs applied locally are good, and so are massages! Ask someone close to you for a back rub, and have him/her use lotion or massage oil- it makes it so much smoother & nicer. Make sure you're getting enough sleep on a supportive mattress & pillow, too.
You can also ask your doctor if you can take some otc pain relievers that won't harm your baby (though most of them can't be taken during the 3rd trimester, except Tylenol). Always consult your doctor before starting to take any medicine while you're pregnant.
Good luck to you, & congrats on the baby!

Have you discussed this with your OB-GYN? You may have an encapsulated uterus. This happens when the uterus is stuck in a retrograde position and is unable to expand with the growing fetus. It can be corrected, but usually will not correct naturally. This would cause severe backache. You should not have to endure that kind of pain at 23 weeks. Speak to your doctor at once so that this can be ruled out. You can also get a prescription for some pain medication that is safe to take. Good luck.
