Can you get one at any doctors office or is there a special place you have to go to? Also, how do I get my drivers background statement? Do I have to go to a police station or what?

No, you can't go to any doctor to get them physical but, there are a lot of offices that do the physical. Just call a few and you'll find one easily enough. The cost is around $100.00 bucks for the first one if you do the drug test at the same time.
Go to the DMV to get your copy of you drivers background.

The National Registry of Certified Medical Examiners which requires compliance this year by May 21, 2014 requires that all commercial drivers see a certified medical examiner listed in the National Registry for their DOT or CDL physical. Motor Carriers will also have to check to make sure that the DOT medical doctor is certified and listed.

You should be able to get the physical at any doctor's office, though I would call to make an appointment and ask if there is anything you need to do beforehand (IE hold your urine for a sample, etc). As for the drivers background statement, I *think* that would come from the police in your town, but I'm not 100% sure. Give them a call; if not, they should be able to direct you to exactly the right place.

You can get a DOT physical at any doctors office. Just let them know that is what you are wanting when making an appointment. To get your driving record, most states you only need to go to the DMV and you can usually get them on-line.

Yes just bring in the form and tell them what it is for!!! Make sure you tell them ahead of time because I belive you need an eye exam and hearing test. They will also have to fill out the form. I think you can go to the DMV
