A nice gift for a professional person could be something like a business card holder, a journal or diary, or a silver photo frame.
As with all gifts, it is best you give it directly to the person if at all possible, accompanied by a card with your personal message inside.
As doctors have a lot of patients, some with very similar names, it's also a good idea to include your name and address on the rear of the card.

I'd say just about everyone would appreciate a gift card or gift certificate to a local restaurant. Leaving an envelope with the receptionist or mailing it in with a nice thank-you note would be fine.

My dentist has these cute figurines and pictures of dental stuff. One of my favorites is a picture of a frog and it says "I didn't brush. I didn't floss. Now all I can eat is mush." Or something like that.
I don't know where he got them, but you can probably find stuff similar at art's and craft sales.
About if you should give it in person or the receptionist? I think it is better to give it to him personally. Unless he is too busy to see you right then then I would leave it with his receptionist.

When my father had heart surgery several years ago, I wrote a thank you letter to the doctor who had CORRECTLY diagnosed my father's illness. If it hadn't been for his correct diagnosis, my father would have passed away sooner than he did (he survived to live 6 more years... we had him with us).
I thanked him for his knowledge and skill, and that even though medicine is a science, there is still a lot of "art" involved in it. I thanked him for allowing my father to live to see more birthdays, more father's days, more Christmases... etc. and being part of our family just that much longer.
He was so very touched by my letter, and thanked ME for my "thank you."
Consider such a thing... maybe accompanied by a plant for the office, or such. When you share your feelings and appreciation, that is a real gift to the doctor.

im working for some paediatricians and there r paintings drawn by children hanging all over the consultation room.
my fave piece: an acrylic drawn glass container for the candies. the kid drew the doc and wrote blessings and thank yous all over. nice!
but then again its for paediatricians.

i think you should give a gift card for a restaurant or starbucks. you could also give tickets for special events / concerts/sports events. movie gift cards are also nice.
