The commonly used tool to achieve a liposuction effect in Photoshop is Liquify. Go to Filter, then Liquify, choose an appropriate brush size and simply drag in the excess fat and skin. Good luck!

Try editing the background to take off the fat ;)

Honestly, there's no easy answer, you can't just magically ask Photoshop to do something like that; software isn't human-like. You'll have to manually cut out the fat using the selection tool or some other variant.

I use resample remove any width/height ratio safety and resample it with fewer width pixels and more height. stop just before the picture looks distorted.
