OMG I sooo do that! One time I can remember looking for the remote control..in my head I could hear myself thinking remote..but the word "apple" kept coming out of my mouth..my kids looked at me like I had two heads..so I said "not the apple" "the apple"while doing the hand gesture for remote..I bet I tried 10 times to say remote and each time the word apple came out..
I'm sure there are hundreds of reasons that that kind of thing happens..over worked brain, ADD, in my case MS.Sometimes my speech gets very slow because I am struggling to make my mouth say the words in my head.But if I relax and don't get all upset the word usually comes.
I wouldn't worry unless you have other symptoms.you might just make a mental note, or jot it down in a notebook, of your activities to see if there is a pattern such as lack of sleep or a medication you have taken.

This condition is called semantic paraphasia - it can be a symptom of some larger problem, and sometimes people's brains are just wired differently, and there is nothing wrong.
Because some of the causes are serious conditions, Its a good idea to see a neurologist to ensure that nothing is wrong (or to catch a problem before it becomes serious). They will test you to determine the cause.

Happens to me all the time. It's basically a form of distractability. In my case, it was caused by hypothyroidism, and seems to be on the mend.
ADHD does it, too.
Especially if the word is something you SEE or sense instead of what you mean....
If I wasn't clear hun, I think you should get it looked at.... but don't panic.
