I've lived in Ontario for quite awhile but I was never given an english name. Now I want to add a middle name but is the name change form (30 something pages) really necessary? My mother said that her friend's daughter added a middle name without having to fill-out the change name form (30 something pages), and that the procedures were much more simpler than the name change form. Is this true? And is the change name form really necessary for just adding a middle name?

So, a "friend's daughter" sounds like someone who might be a child changing their name which is a different process than an adult changing their name. So one factor is how old you are.

Click on the link and in the second paragraph you can click on the change of name wizard. It will ask a few questions and get you to the right forms.

Yes it is necessary to fill out that form - sorry - but it is the only legal way of changing you registered name.

i think you need to fill up a form to make it legally binding
