The poor thing is scared to poo now. The vet doesn't know what is wrong with her. There is nothing stuck in her intestinal tract and only seems to hurt right at the point of exit. Oily food helps, but not 100% of the time and I seem to need to give her more and more oil for it to help. The vet is trying her on anti inflamatories at the moment but it isn't helping yet. The vet has squeezed her anal glands a couple of times, and it doesn't hurt her so it doesn't appear to be those.

Yes she is still young, only 15 months old. The vet is almost ready to send her to a specialist and to get an endoscopy ($$$). I will try another vet then and mention the fissures. Thank you!

It isn't a fibre issue ... her poo is fairly small and soft.

She has anal fissures probably. I'm surprised your vet doesn't know about this as they beyond common.
She needs something to make the bowel movements soft to give the fissures time to heal. He can also prescribe something like preparation H that basically numbs the anal area so it doesn't hurt when she goes. Otherwise she'll retain and the problem will get worse. The only trouble is that you have to use an applicator to put the stuff in her anus three or more times a day at intervals to keep the area numb. Once the fissures heal you need to make sure she has soft stools for awhile as the area is still prone to cracking. After she's good and healed it'll go away.
This usually happens to young and small dogs. If she's young it will get less of a problem as she gets older.

I suggest taking her to another vet because if this one hasn't heard of anal fissures in dogs, something's wrong. I'm not a vet either so I'm not wanting to "diagnose" as this would be wrong. I just think he's a dolt for not thinking of the most simple thing.

AND GRAPES ARE TOXIC to DOGS. So are onions.

If she looks at her behind randomly as if there is a sharp shooting pain, or tries to chase her tail then perhaps she needs more fibre, canned pumpkin or bran flakes might help?

Give her some canned pumpkin, not pumpkin pie mix stuff, just plain canned pumpkin. It helps with diarrhea and constipation both.

mabey, as gross as it is to say it, the poo feels spikey. My little brother was pooing one day and when he was done, he was crying and said "it feels like i have nails in my poo-poo" so mabey thats it.

to cure my brother we gave him grapes. But idk what to do for your poor puppy. : /

Don't give your dog grapes. They are poisonous.
