The two people are not arguing or anything, one just knows the other one has something bad going on but the other person "doesn't care to.." etc...

Either it is a subject about which they feel very strongly and they do not want to rock the boat, create a conflict or go into details because it is too personal, or they think that it is a waste of time as the person they are talking to will react negatively, will not shift from their position, or will be bored or offended.
PS. Ref. what you have just added: In that case it sounds very much as if it is a polite way to say, "yes, things are bad, but it is too painful/ awkward/ private to discuss and it need not concern you".

It means the person disagrees strongly or is concerned that the disagreement will become socially or emotionally threatening. It's generally used when a subject is very emotional and the person saying it does not want to get angry or be exposed in some way.

I've heard it when the person saying it was being confronted with an embarassing or damaging truth, or when a third party was present. It basically means you're asking me a question or making a statement I don't want to answer, and I am trying to be polite and/or nonconfrontational about it. It's less aggressive than saying "back off".

It means that the person doesn't want to open a conversation about a certain topic. The reasons can vary, but the meaning is the same.

Depends on what the subject is. I believe that most of the time it's just the inability or disinterest in dealing with a problem.

It means they don't want to talk about it for whatever reason. They aren't giving the reason, so all you know at that point is they don't want to discuss it.
