In Hellraiser 2: Hellbound, the cenobites are destroyed by an indescribable demon. Though the scene wasn't terribly gory, it was moving. As they die, the grotesque cenobites return to their original human forms: all 4 of them appearing as good-looking, attractive people. You begin to wonder what kind of awful mistake could have transformed them from beautiful to hideous; could they really have deserved such an evil fate?... I felt sorry as hell for them.

I liked the death in the hills have eyes 2 (1985), when the guys sees a trap, and successfully avoids it, only to shout in triumph and initiate another trap. During his shout of triumph, the other trap drops a boulder on his head. That was pretty funny.
The most painful to watch (for guys) was the last torture in Hostel Part 2. Lets just leave it at the fact that it was a graphic castration.
Another good one was in the movie High Tension. A guys gets his head shoved between the rungs of some stairs (so he cant get his head out) and the killer takes a desk or something and rams it into his head (decapitating him).
PS You get a star. I love this question

The scene from ANDACONDA where John Voight gets swallowed whole by the giant snake, then puked back out in a later scene, all half digested (yet still alive).
The scene from SCANNERS where a guy with ESP causes his enemy's head to explode all over an audience before he could spill the beans. (First exploding head in the movies).
The scene from DEEP RISING where most of the mercenaries who came to rob a luxury liner get slowly devoured by giant undersea worms who previously invaded the ship. Slowly digested while still alive.
Uncut scenes from the original KING KONG (1933) where Kong is graphically squashing villagers with his foot and some stick to his feet like stringy gum.
The scene from THE PROPHECY (1979) when a 25 foot tall toxic waste mutated grizzly bear swats a camper trying to hop away inside his sleeping bag into a tree. SPLAT!

I know Irreversible isn't generically a horror flick but it's use of a fire extinguisher as an instrument for caving someone's head in makes most horror movie deaths laughable.

Id say the Saw movie deaths or Hostel or nightmare on Elm street. They were pretty bad from what I could tell.

I am fond of 13 ghosts when the guy is sliced in half by the sliding doors, also Saw 3 when the guy is strapped to that device that turns all his bones and finally breaks his neck.
My favorite is Jason X when he dips the docs head in that freezing stuff then shatters her head like a glass on the counter!!!
