Robert Koch 1843-1910 : German bacteriologist who came up with a system - "Koch's postulates" - for determining microbial origins for common diseases including tuberculosis. This was a huge advance, but this would have saved few lives without the discoverers of effective antibiotics such as Alexander Fleming and penicillin. Of course Lister and Pasteur would have to be on the short list.
If you want to go back further, consider Edward Jenner and smallpox vaccination in the late 1700s.
DO NOT choose Galen - Roman physician in the second century CE who recorded nonsensical sure fire remedies that unthinking physicians would blindly follow for more than 1500 years. Hippocrates was OK, but the late 1800s and mid 1900s guys were much more important for improving our chances of longer lives.

There are too many to mention!! It really depends on the field of medicine you are looking at, personally I would say in the last century Alexander Flemming - he discovered penicillin and so pretty much discovered antibiotics, which changed the face of medicine and how we treat disease. But I am biased as that is my field!
