The host calls to invite (if it is casual - if formal, they send an invitation). If the party accepts during the call, then that is all that needs to happen. If the party has to check their schedule to make sure, it is up to them to let the host know.
If it is an invitation, include RSVP on the card - the invitee should respond via telephone, except in the most formal of occasions, which merit a written acceptance.

When invited, etiquette requires that the invitee confirms as early as possible preferably through a phone call or through mail as required by the hostess. When the invitee does not respond, the hostess is obliged to call to confirm whether the person is attending the party.

I dont think it matters who confirms...as long as someone does it.
If I am the hostess, I call 24 hours ahead of time to confirm...(so I can buy groceries that wont go to waste.)
If I am the invited guest, I call 24 hours ahead to keep from wasting a night that I could other wise plan to attend another event.
I think 24 hours is plenty of notice if you cant attend or need to cancel.

But if the host does not call to confirm, I see no problem with the guest calling to make sure that the plans are still on.

The host or hostess generally does everything. A confirmation usually isn't necessary, but there isn't really any protocol if you feel that it is necessary.
