you need to have wage slips birth certificates of your kids details of your partner if living with

Birth certificates for any children named on the claim along with the Child Benefit number. Your previous year's P60 details unless you were on income support, income based job seeker's allowance or minimum income guarantee. Any taxable social security benefits will count as income.
Proof of any disablities for yourself or the children should you include any on the form.
You should also make sure your address is correct and more importantly, whether you have the recourse to public funds/right to reside.

Income for the previous year (to 5/4/07)unless you are on benefits, DWP will disclose that info.

Your partner is only included if you live together

You may need proof of your benefits income. bank statements etc.
They are only interested in your income/savings and how many children you have.
