If there is a problem that is serious enough for you to want to see a Dr about then just make an appointment and go there and tell him/her what is going on it your life...they have heard it all and aren't there to judge, only to help. Get help as soon as possible and good luck.

Just make an appointment do not need to state why, and before you go write down all the reasons why you feel you have anxiety/depression, then if feel unable to explain yourself when there, just hand over written information. This is always a good idea anyway, even if feel like talking may leave out the little details, which really can make a difference in getting right diagnosis.

You really should go because if you have depression and/or anxiety, it could be from another underling condition which has not been dealt with. Depression /anxiety just does not disappear so will eventually get worse if not dealth with, so go now as the longer you leave it the harder it may become.

Just go in there and explain your symptoms. He will probably write you a prescription for antidepressants and anxiety medication. Don't be afraid to stand up for yourself - the doctor deals with this every day. That's what he's there for. Good luck!

You say, "I'm depressed and I have anxiety and I'm tired of it. What can I do?". Seriously, that's all you have to say.
