Because in order to learn from your mistakes, you need to make some essential changes to yourself, to your psyche, and how you view and react to things. Maybe they do not learn because they do not suffer enough for the mistake they have made, or maybe they think it is going to turn our differently next time. Maybe they don't accept responsibility for their mistake in the first place, and keep blaming circumstances and other factors. Once you accept responsibility, and do an analysis of how that mistake has had serious repurcussions on your own life or of those around you, you would reform.

It strikes me that some ppl just don't realize what happened and how a disaster occurred. So, if they cannot define what the problem is, they cannot devise a solution to avoid that problem in the future. It reminds me of an episode of Hogan's Heroes where Col. Hogan was diffusing a bomb that had been dropped by an Allied bomber (accidentally into the middle of Stalag 13). Col. Hogan saw there were 2 wires. A blue one and a red one. He told Col. Klink if he cut the correct wire, the bomb would be diffused. If he cut the wrong wire, they'd be blown up. He asked Klink which one he should cut. Klink said "cut the red one." Hogan cut the blue one. Klink: "If you knew which wire to cut, why did you ask me?"Hogan: "I didn't know which one to cut. But I knew that you'd pick the wrong one." Some ppl are just not good at analyzing mistakes so they repeat them (not just a few times, but sometimes 4, 5, 6 times). Yeah, it can be frustrating. I also think that early upbringing might play a part. No, I'm not blaming all of the parents. However, if both parents are silent and never teach their kids anything about life (how to keep a budget, how to have good manners), the kids are going to have a hard time. I know a kid who was absolutely clueless about nearly everything (didn't know how to wash his hands with soap and water, didn't know how to match clothes, etc.) and he was an older teen.

If we never repeated our parents and their parent parents and so on we would have been so advanced but it is the cycle of life. Don't you hear kids always saying i have to make my own mistakes? Only many of the mistakes are the same ones the the parents made.

So to your question the reason people don't learn from their mistakes is because of the cycle of life, some people think they can get it right if they only try again. Go figure???

It is a logical thing to learn from one's mistakes.
Most people are able to devoid themselves of logic in order to satisfy an emotional want or in avoidance of a trumatic emotional reaction. It is more comfortable to lie to yourself than to work for betterment.
Add a negative outcome, as a result, and repeat the lie....
This defines INSANITY.

A world ruled by lies, An insane world.
