If you enlist than you won't be a doctor, you'll be a medic or a physician's assistant. If you try to get a commission, then you won't be able to get a commission for being a military doctor unless you have doctorate in medicine.

Doctors are Direct Accessions and as such, you are required to have completed internship before being selected for a Commissioning. If you enlist, all training will be provided, but assuming you go in as a medic, you will NOT be trained as a doctor. think glorified EMT.
