It depends what country you are a citizen of. I'll answer for the US, however.

To get a passport you need to fill out the appropriate form (see link below), get proof of citizenship (normally an official copy of your birth certificate, but it can also be a record of birth abroad to American Citizens, Naturalization papers, and expired passport or a couple other more obscure records). You also need photo identification (a drivers' license is almost always sufficient for this; military IDs and other official IDs with pictures also work), plus the pictures and the fee.

The form asks for your personal information (name, birth date and location, address history, etc.) for you and your spouse, your travel plans, and a few miscellaneous questions. It depends on whether this is your first passport or a renewal, also. The info I have put above is for someone getting a first passport.

You are also asked the names and birthdates of your parents, and if you are married the name of your spouse. Also your SS#. The application also asks for any other names you have ever gone by..including nicknames.
The gov. actually does a background check on you. If you are a convicted felon, you cannot get your passport, if you are way behind on your child support you cannot get your passport. So as long as you have a clean history and can prove your citizenship in the USA you will be able to get one.
These answers are for citizens of the US. To check out the form to fill out you need the DS-11. Below is the link to the form you have to fill out.
