In my state there really is no "fork truck license". There is no agency that controls or issues them. There is no form or format for one. Permission to use a forktruck is normally called a license, and is granted by the employer based on their own criteria. This usually (but not always) includes having a valid motor vehicle license. My employer requires re-certification through their program on an annual basis.

First, it depends on what state of the US you live in.

OSHA requires retraining every three years, unless new equipment is introduced or you change employers.

Some states require more frequent training. California requires retraining every two years. Several other require similar.

There no longer is a 'liscense' for forklift operations, but every company that uses them needs to have training records showing the correct training. It needs to be administered by qualified individuals, and that also needs to be documented.

New equipment or new employers/employees also require special attention.

Whenever a new piece of equipment or a new attachment is introduced training on that specific equipment needs to be done and documented.

When new employees start, or you change jobs special attention needs to be given. The company has the option of not retraining an employee that was previously certified by their former employer as a lift truck operator. but they do need to have documentation that this employee was certified by whomever they have do their certifications.

OSHA has no requirements as to whether a lift-truck operator needs to have a state drivers liscense. That would be a specific employer's requirements.
