The prayer for forgiveness of all your sins, past, present and future, is to ask Jesus to forgive you of all your sins and accept Him as your Lord and Savior. Once you are born again the Holy Spirit comes and lives in you convicting you of your righteousness in Christ every time you sin so that you are encouraged not to sin (John 16:10) and you are put in Christ by God (1 Corinthians 1:30). After you are born again you have God's forgiveness everytime you sin because you are, spiritually, in Christ all the time and it is in Christ that you have the forgiveness of sins (Ephesians 1:7; Colossians 1:14). 1 John 1:9 is actually a prayer for salvation because it is only when you are saved that you are cleansed from all unrighteousness. We have God's abundance of grace and the gift of His righteousness to reign in life (Romans 5:17). When we are in grace sin has no power over us (Romans 6:14). To have victory over sin it is not a matter of asking God for forgiveness but to walk in grace and truth and in Jesus and be led by the Holy Spirit (Galatians 5:16).

Since God is everywhere and sees and hears everything, when you asked this question to us, you also asked this question of God, didn't you? Just talk to God. That is a prayer in itself. God understands things we don't or can't, anything you ask God, while awake or sleeping is kept by God and will be close to your heart. The answers you get or have trouble interpreting may vary, but the answers are there. God does not give you the answers you are looking for, but the answers you need.

Only God can understand the language of feelings. Share everything with him, be as God want's u to be. God will forgive if you truthfully accept your mistakes, tell him and never repeat them again.
Who is God?
Who has been remembered as the Ocean of Love, the Intellect of the Wise, the Almighty Authority, the Comforter of Hearts, the Truth, The Living Being and the Blissful One?

You need to pray what comes to your heart. That will be a true prayer asking for forgivness.
Here is one just to get your head and heart going in the right direction:
My Dear Lord Jesus Christ I am coming to you with my heart and soul open for your healing touch. You know I have sinned and I come to you asking for Your forgivness. I know that this forgiveness that I am asking for can come only through you to God who has the wisdom and power to do that. My past was rocky but I want so much to make it smooth by Your forgiveness. The Bible says:

I will tell you a unique thing.
Buddha never taught others to pray to God.
He asked people to become.
Imagine what will you do , if you are God.
Then all will be like your children and you will
think that :
"May all beings be happy.
I forgive all, anyone who had done any wrong to me.
I ask for forgiveness for all wrong I have done to anyone.
May all creatures be happy and satisfied,
I wish well being for all."
By doing this you are a giver
not a beggar
and giving is same as receiving.

What you need to do right now is pray to God from your heart and confess your sins before God. Don't pray someone else's prayers, pray your own. Tell God that you have sinned and ask Him to forgive you. Ask Jesus into your heart and to teach you what He would have you do for Him so that you may now live for God. God will forgive you and give you that new life that He has promised to all who confess their sins and repent of them. Just do it now, from your heart. May God give you wisdom and knowledge to know and do His will.

We once did this beautiful ceremony at church camp when you wrote down your sins on pieces of paper and then in a group burned them. You literally watch your worries and sins burn away.
God is forgiving, at least the Christian God I'm assuming you're talking about, and I think you need to just talk to Him/Her and say that you want to repent for your sins.
If you're Catholic, there's the whole confessional thing, which I'm personally not too down with but these all help.
Whatever you do, YOU are the one that is left with the feelings of guilt/redemption, so do what feels right.

Psamls 51...However u have to be open to God and tell Him what u need forgiveness for, just speak to Him and be honest and sincere. Pslams 51 should help you to start if u dunno how. God is a loving and forgiving God, there is nothing that you have done that is too terrible for God to forgive. Be blessed!
