Depends on what it is? Usually you should start with your supervisor.

The biggest issue with some things is it can get you into a lot of trouble if you don't report certain things. Child abuse for instance you are a mandated reporter. Many just pass it on to their supervisors hoping it will get reported. If it doesn't, you are responsible, and you can get charged criminally!

If it is medical, I guess your chain of command, however many go no further than the one you reported it to!

Talk to your Charge nurse or the floor administrator to find out what your hospitals protocol is they will tell you what steps to take.

Follow you chain of command. If you feel like you can't talk to your immediate supervisor go to the next higher one.

If you are a nurse..you should already know. I would not want you as my nurse if you don't already know..trust me..i am a nurse. I am not helping you because you should know!! Geesh!!
