Why are most flight hours logged while a pilot is ranked a captain?

There is a monthly limit of flight hours a Pilot / NFO /Aircrew can fly based on the type of aircraft they fly. It's something close to 65 hours for jets / 100 hours for Helo's or Prop's / 120 hours for pressurized aircraft. They may fly more hours in a month after flight surgeon approval. There is no career limit on flight time. The reason why the majority of flight time is while they are a O3 is the fact that it takes 6 years to advance. They will make 2+ full deployments during that time when flying is their primary job. After they make O4 they tend to move away from flying as much and against their will start working the desk more.

Nope, as long as a pilot can pass their flight physical, they can keep on flying.

But they do fly less, once they get to squadron commander rank.

The higher your rank, the more non flying duties you have.

Pretty sure there is a minimum. A max number would depend on the fuel budget most likely.
