There are 2 possible visa categories she could apply for, either the H1B, as I assume you have, and already understand the procedures, or the H1C. However, if she hasn't already taken and passed the CGFNS or the appropriate state licensing exams, she'll need to do so. The old H1A category for nurses no longer exists. the H1C was established for nonimmigrant nurses who will serve in certain medically under served areas. According to the State Department regs, available through travel.state.gov, these are the requirements:

(1) Have a full, unrestricted license to practice in the country of his or her nursing training or have been trained in the United States;

(2) Have passed the examination given by the Commission on Graduates of Foreign Nursing Schools (CGFNS) or have a full, unrestricted license in the State in which the alien will practice, or in any state or territory of the United States and have been given temporary authorization for employment in the state of intended employment; and

(3) Be fully qualified and eligible for employment immediately upon admission.

I do not know what an H4 is. However, she needs to reside for few years in your state, until she is eligible for a green card. I remember, I had to stay here for three years. But laws since than have changed. So best thing check with your local INS . prepare paper work and file and see what happens. Good luck.
