How To Apply
Application is a simple process. You should use a DD Form 293, Application for the Review of Discharge or Dismissal from the Armed Forces of the United States. In addition to downloading the form, The DD Form 293 is available at most DoD installations and regional offices of the Veterans Administration, or by writing to:

You should complete the form very carefully by typing or printing the requested information. Where to Send the Form
Mail the completed DD Form 293 to the appropriate address:

You need to make application to your service's board for correction of military records. But, you have to come armed with evidence that the original decision of the command which issued you that discharge was in error. You might be able to get some help from your local American Legion or VFW post.
