First be very careful and review whose fault it really was. Dont forget that health insurance carriers unlike home or auto are not controlled by the agents therefore it may not be the agents fault at all. Agents usually are just a third party and if you think that perhaps there was miscommunication then take it up with the agent first. As for reporting them, depending on insurance company they represent, you can complain to their district manager first, or call corporate offices.

You would contact the insurance department for your state. You should be able to find a web site for them. You can usually file a complaint via internet or phone.
All insurance companies (health, life, auto, homeowners etc) are regulated by the insurance department of the state they are doing business in.

You should NOT complain to the Illinois Insurance Commissioner, unless you live there. You should file the complaint with the Insurance Commissioner of YOUR state. That commissioner regulates insurance in your state...which your policy is under.
