Quite frankly it's because the oral cavity all the way to the anus is "open" to the environment. While it does internally traverse the length of the abdomen, the GI tract is still considered to be external since it is open at both ends to the external environment.

Internal body cavities are those that are completely walled off from the external environment by well defined structures such as skin, mysentary, etc. In contrast the GI tract is specifically designed to periodically open to the external environment an allow food to enter and waste to leave.

Demons are not bound by bodily obstructions that would be in the way of material things. They are spiritual, therefor, they enter into the spirit not necessarily the body, though they can control it. Demons do explain a lot of the witchcraft and rituals that have produced "supernatural" happenings. They can and will do anything they can to deceive people, in fact, they can make themselves into fleshly beings (see Genesis 6). It isn't something to be taken lightly.

The mouth and the anus are the termini of the alimentary CANAL. In almost any sense, it is not a cavity.

Is the hole in the middle of a donut an internal surface or an external surface? Goes the same for your mouth/butt babe. depends how you look at it.
