Could be a few different things, including the following:

Radiator clogged,
Cooling fan motor bad
Cooling fan relay bad
air in the system
water pump bad
hose collapsed and blocking the flow
bad coolant temp sensor or switch not turning fans on
blown head gasket
cracked head
cracked block

If this is a 3.5 Lt., there is an air bleed on top of the thermostat housing. ( Top front of the motor) Loosen, but don't remove it. Let the motor run with full radiator until all the air is out an nothing comes out but coolant. You can also purge air out with the engine off by pumping up the coolant reservoir with a pressure tester with the bleed valve open. Then make sure the radiator fan(s) come on. If it runs hot out on the highway, have a mechanic check the gas in the radiator for exhaust fumes, which will indicate a leaking head gasket. You can buy you're own exhaust gas tester at a auto parts store. Good luck.

Bad fan, it's not kicking in when the heat rises in the motor!
