if it is not a family member then you do not need a proffesional person to do it.i know as i had a friend of mine to sign.They do ask on the form if you have known the person for 3 years and they ask their passport number.i had all this and had no problems

I do not think so. A teacher or professional person yes but I do not think an assistant teacher counts as a professional; person

the person that does sign it must have a full current passport as they have to fill in a form with their passport number on it.

your doctor can do it but they usually charge a fee its quite a bit .I got my dentist to do mine and it cost me nothing just a case of looking.
