That last answer I read was absolutely ridiculous. When a women has sex she has sex. There is no change in the size of anything and there is no way for a man to tell if she had sex with a guy yesterday or ten guys yesterday...

Yes it is quite possible for the regular to know. There may be some factors such as time in between the two as well as the difference in size between the regular and the other. Some guys really pay attention to things like that and notice and some don't.

If he breaks out in a rash, discharge, fever, chills, and other STD symptoms a few weeks later, then yes, he will notice a marked difference "down there," definitely.

He won't know you cheated on him until the lab tests get back and he finds out what disease you gave him because you messed around. If he didn't cheat on you he will know what you did and will never trust you again, and with good reason...
