When I do that it usually means I like them a lot! And I would go up to them and say something simple but sweet! Then start engaging in conversation! If you already know em good, ask him to go somewhere, and if he says no then at least you know and you could move on.

I think it means the obvious, that you really, really care about them, whether it be a close, platonic caring, or a more intimate feeling, you are caring about them beyond the explanation of words. Talk to them, take a chance and let your feelings be known - they can't be recognized or returned any other way.
