For the most part, no. There are upgrades that can be monthly expenses, such as real-time traffic data or similar services. You don't need these to have GPS, though.

The cellphone based GPS do require data packages that add expense to your cell phone bill and the maps are pretty small, so i'd avoid them.

No the GPS system is for now, free the system was set up for the govt mostly for military applications. It has grow to many applications including cell phones.

No, but every couple of years you'll have to buy a map update.
