1. If its her second marriage...
2. If she has reached a certain age (differs in different countries)...
3. If her father has passed away...
4. If the father doesn't have any good reason for not giving permission...
The permission thing is kinda in her own good interest... cz the father knows much more about men and their intentions...
BUT... if the father doesn't give permission without a good reason... then she can get married by the judge to the person she wants... :)

In order to marry, a Muslim woman has to have the permission of the close male relative who is responsible for her support and protection.
If this person refuses to agree to the marriage, and she feels he is being unfair, she can ask a Muslim judge to consent to the marriage, instead. I know people who have done this.
Muslims do not date. A Muslim couple does not touch, or see each other dressed informally, or see each other privately, until they have made a public, lifelong commitment to each other.
Obviously, when a man is alone with his buddies, he's going to behave differently that he will when he's all dressed up and going courting. Only another man can get to know him well enough to know for sure that he's a good marriage prospect: honest, respectable, hard-working, sober, religious, and kind.

scholars differ, most muslim countries give women the right to marry legally since shariaah law isn't practiced , and even where it is practiced islamic judges may have different rulings
and can act as a guardian if the father or other guardian has forbidden marriage without a valid reason .
i believe that in most cases a woman better have her family's support and approval because marriage is a union of two families and no man is worth losing your loved ones over .
if the family doesn't care for the woman's future and just want to control her then she can easily Seek guardianship from the court or other relatives with the help of the court to prove the man is fit for marriage so she can marry as she wishes.
i know muslim women who married against the wishes of their families in islamic countries but most of them regretted it because love marriage is usually not thought through .

An adult Muslim woman is the master of own destiny. She can sign any contract, including a marriage contract. It is customary, in the interest of keeping family system intact and growing that a lady obtains the good wishes of her family elders before contracting a marriage... or accepts suggestions made by elders, as in arranged marriages.
