1. patient was implanted with a pacemaker 2 years ago.
2. 2D echocardiogram was performed recently
3. the sonographer was surprised that there was a pacemaker, since the patient stated only during the proceedure that he has a pacemaker.
4. Human error may be a factor but please enlighten me if it can be detected.

Reason for this inquiry is that I would like to know if it is indeed detectable by 2D echo why was it missed if the one who performed the proceedure was surprised there was a pacemaker and didn't see it. Thanks

Echocardiogram performed by placing the transducers on chest called transthoracic echocardiogram may not image pace maker properly. As the pace maker is placed at the place of dysfubnctional SA node located very near the end of superipor vena caval opening into right atrium.
A transesophageal echocardiogram better visualises blood vessels close to heart.
