Why haven't you taken your dog to your vet? It could be cancerous.

This is a Vet visit required to make sure that this is some abnormalty of something, this could very well be a start of a TUMOR, in most case could just be big CIST inside the Nipple, Tumors can be common depending on the history and genetics, its best if it is to remove it as early as possible,
CIST, in the nipple can be a possibility, just give it a little sqeeze if something pops out than its a cist (ZIT) in the dogs nipple if so just keep pressing untill it all comes out
if the vet finds it as a tumor, and he removes it
i would suggest to get your dog on some vitamins that will prevent future tumors,

There could be an infection in it or under the skin.
It's called an abcess. Pus pools under the skin and causes swelling.
Blood pools like that too. It's called a hematoma. It's caused by injury. Even mild injuries can cause hematomas.
Older dogs have lumpy fat tumors
It could be any sort of tumor/cancer.
Basically, you need a vet ASAP. Fast treatment is needed for complete recovery.

Male dogs nipples should remain the same size once he's full grown. If it's just a single nipple, take him to the vet. It's probably just an infection of some sort.

Things that can be considered:
like stated by others: absesses, hematomas are quite common.
however, tumors are something not listed yet.
in any case: take him to the vet. hopefully, depending on breed of dog, like labs, most tumors are benign. but never take a chance. go and have it checked out. the sooner the better.

Did you ever think to ask your vet this question? It could be a cancerous growth which will eventually kill him, then again it could be nothing. I doubt very much if anyone here knows the answer. Take him to the vet.
